K-2nd Grade (ages 5-7) – 15 students per week, $135 per student

3rd - 6th Grade (ages 8-11) – 20 students per week, $230 per student

10th - 12th Grade (ages 15-17) – 15 students (total – both weeks), $375 per student

Application & Forms: (All forms are Adobe Acrobat files so you will need Acrobat Reader to open and print these files.) Please fill out the application form, information form, and indemnity form for all students who would like to attend this year’s camp. Payment can be made via PayPal or you can mail a check/money order directly to the Chorale at PO Box 2103, Evergreen, 80127-2103, attention “Summer Music Camp.”
